How do I cancel an appointment with Appletree?
Enter the patient’s name as it appears on the health card or government-issued ID. We will contact you by email to confirm the cancellation or rescheduling of your appointment. Provide the date of the appointment that you wish to cancel or reschedule. YOU MUST PROVIDE THE CORRECT APPOINTMENT DATE (MM/DD/YYYY).
